The Office of Facilities takes care of the following issues:

- Hot or Cold Portion of the Building

- Plumbing Leak and Plumbing Stoppage
- Roof Leak, Gutter Leak, Floor Drain or Light Well Overflow
- Elevators
- Pest Control
- Moving Requests
Please read the instructions on how to complete the maintenance request form before filling it out, so that we may provide the best and most efficient service.
Maintenance or Repair Requests
For Maintenance or Repair requests, the following information needs to be provided on the TDX ticket:
Name of person requesting work
Library or area of problem
E-mail address
Phone number
Precise Description of problem
Room number of problem
If you need a picture, whiteboard, etc hung or attached please put the location of the item needing hung or attached and the location of where it needs to be hung or attached.
If the information above is not completed there may be a delay in processing your request.
Elevators
Elevator issues should be reported immediately via phone
For Elevator requests, please call:
(Day 8am-5pm, M-F ): Library Facilities: 217-841-7874
(Night and Weekend): Facilities & Services (non-emergency Police Dispatch) 217-333-1216.
Be prepared to provide the following information:
- Name of person requesting work
- Name of departmental library or unit
- Phone number
- Name of building, elevator number if known, or approximate location of elevator in the building
- If possible, whether there is a person trapped on the elevator.
- Precise Description of elevator problem
- If the elevator is stuck on a floor, please give the floor it is stuck on.
The information above is critical in processing the request quickly.
All elevators are equipped with Campus emergency telephones that report to Public Safety Dispatch.
Upon completing the call, please submit a TDX ticket with the list of information above for Library Facilities to follow up with Facilities & Services.

For temperature requests, the following information needs to be provided on the TDX ticket:
Name of person requesting work
Library or area of problem
E-mail address
Phone number
Precise Description of problem
Room number of problem
If this is a Radiator problem please give the location of the radiator in the room and the location of the closest thermostat.
If the information above is not completed there may be a delay in processing your request.
Window A/C
For Window A/C requests, the following information needs to be provided on the TDX ticket:
Name of person requesting work
Library or area of problem
E-mail address
Phone number
Precise Description of problem
Room number of problem
Type and Model of window A/C
Inventory number(if applicable)
Last service date(if known)
If the information above is not completed there may be a delay in processing your request.
Light Bulb Replacement
For Light Bulb Replacement requests, the following information needs to be provided on the TDX ticket:
Name of person requesting work
Library or area of problem
E-mail address
Phone number
Precise Description of problem
Room number of problem
Please put down if this is an incandescent or a fluorescent bulb and area of the room it is in.

Plumbing Leak and Plumbing Stoppage
For Plumbing Leak and Plumbing Stoppage, the following information needs to be provided on the TDX ticket:
Name of person requesting work
Library or area of problem
E-mail address

Precise Description of problem
Room number of problem
Please give the room number and sex of the restroom in need of service.
If this is a restroom in need of cleaning or out of paper products please send a maintenance request.

Roof Leak, Gutter Leak, Floor Drain or Light Well Overflow
For getting Roof Leak, Gutter Leak, Floor Drain, or Light Well Overflow requests, the following information needs to be provided on the TDX ticket:

Precise Description of problem
Room number of problem
If this is still leaking and in need of a clean up please call direct to the facilities office.

Pest Control
For Pest Control requests, the following information needs to be provided on the TDX ticket:

Precise Description of problem

Moving Request
For Moving requests, the following information needs to be provided on the TDX ticket:
Name of person requesting work
Library or area

Precise Description of move needing to be done
Inventory numbers on furniture or equipment (if applicable)
Furniture or equipment needing removed
Inventory numbers of furniture or equipment needing removed
If you are moving to another room:
Please provide the Jack number and telephone number you are currently at.
Also provide room number and or building you are moving from and to.
